Clinician Testimonials: How integrated care helps save patients

We receive feedback every hour of every day from our patients and their providers telling us about the impact we have on behavioral health care. We occasionally publish testimonials that illustrate the way we support both providers and our mutual patients. With permission, we’re sharing the recent testimonial of a provider whose elderly patient was at risk and needed a timely intervention…

“Amanda helped save the life of our mutual patient by promptly alerting me that a patient did not answer their scheduled concert health phone call. The patient had recently brought up concerns of worsening anxiety, depression, and passive [suicidal ideation], and Amanda was correctly concerned for the patient's wellbeing. After receiving her communication, I asked our nurse team to call for a wellness check. [The police department] found the patient confused and she was transported by EMS via ambulance. She was unresponsive by the time she got to the ED and was intubated. A few days later she was extubated and is now recovering in the ICU. The patient and her daughter credit the wellness check with saving the patient's life. I wanted to make sure Amanda was acknowledged for her crucial role in escalating this.”

The Concert Health team was grateful that we could intervene in time to help this patient avoid a much worse outcome. We asked the clinician to share her perspective as well…

“I am proud of the role I was able to play in supporting this patient, and this experience was a powerful reminder of the impact that strong collaboration can have on patient outcomes. While I’m grateful to have been involved, the positive outcome we saw was truly made possible by a highly responsive provider and care team. The provider and their team acted quickly to attempt to reach the patient and were proactive in their efforts to ensure her safety. When the patient did not respond, they made the important decision to request a welfare check, demonstrating a deep commitment to patient care and wellbeing. Moments like this highlight collaborative care at its best, when providers and care teams work together, communicate effectively, and act quickly to support patients when it matters most.”

By integrating behavioral health and delivering multiple virtual touch points to patients, care teams are more likely to identify patients in need of acute intervention and avoid much worse outcomes.
