Our Collaborative Care Model

Concert Health helps your practice take a population approach to behavioral health. We identify patients through screening tools, prescription data, and provider referrals.

A member of the Concert Health team will schedule time with the patient for the clinician to contact them. Our clinicians directly connect with patients, who can choose to engage via video or telephone. The clinicians work with patients to set goals, provide counseling, monitor medications, and regularly assess symptoms.

Each week, Concert’s Psychiatric Consultant reviews those patients who are not getting better, providing treatment recommendations directly in the PCPs medical record. They are also available for curbside consults.

Concert Health makes sure you can bill and collect for Collaborative Care, which is a covered benefit for Medicare, commercial insurances, and Medicaid in an increasing number of states.

Together, Concert Health clinicians and PCPs treat patients and build key relapse prevention capabilities/strategies, empowered by Concert’s technology platform to help scale this excellent Collaborative Care.

We are seeing great success in remote areas where we are able to provide care to those who may not have access to a specialist clinician or resources. This model, combined with the right technology and connectivity, is allowing us to bridge the gap and provide accessible care to many who would not have had access previously.
Patty Morrow, VP of Behavioral Health Services, Mercy

It has been a dream come true to embed this facet of patient care within our offices. Patients visibly relax with relief as they learn behavioral health professionals practice within the same location. The willingness to engage with our psychiatrists or psychologists is almost 100% now, contrasting with almost never in the past.
